Mayday: Air Disaster - From Season 9 Episode 8 "Cracks in the System":Chalk’s Ocean Airways Flight #101 — Grumman Turbo Mallard (sea plane)
Destined for the Bahamas, Chalk’s Ocean Airways Flight #101 is preparing for takeoff in the waters off Miami Beach, Florida. Part plane, part speedboat, the twin turbo-prop taxis through one of the world’s busiest ports, and after a smooth takeoff, climbs towards the clouds. A minute later, however, just as the plane is leveling off, it is suddenly rocked by an explosion and, from the beach, a tourist films the aircraft’s catastrophic plunge into the ocean. Lifeguards immediately race to the crash site but there are no survivors. Eyewitness reports and the tourist video confirm one of its wings ripped off in mid-air. But can investigators determine why?

In the world of Air Crash Investigation, some of the most crucial pieces of evidence can be found inside an airplane’s flight recorders. Having a recording from on board an aircraft ready to be retrieved to tell the events of an ill-fated flight is invaluable to any investigation. They can tell some remarkable things, and sometimes investigators can get a whole lot more than they bargained for. We’ll be talking about one such case today.
When you think of the term Flight Recorder, two different objects may come to mind. There’s the Flight Data Recorder which records various parameters and technical data about a flight, and then there is the Cockpit Voice Recorder, which does exactly what it sounds like. One or more microphones positioned in the flight deck recording the conversations of the pilots.
Together these two items, though they are known colloquially as “Black Boxes” they are typically coated in bright orange. Done so to stand out amongst plane wreckage or at the bottom of the sea. As you’d expect therefore, given the sensitive nature of the voice recorder’s content, these aren’t always released to the public unlike say accident reports or photographs.
Sometimes they are for various reasons if it is in the public interest, some countries have been known to release these recording more liberally than others. In other cases the recordings themselves are withheld but they are transcribed.
Today though we are going to look into the case of Braniff International Airways Flight 352. This is a rather rare case of an air accident. Where though the disaster took place 55-years-ago and for almost all of that time the cockpit voice recording was kept from the public, just two months ago at time of recording this video, it surfaced. Though the recording was withheld, I’d say having looked into it, I’d describe it more as lost.
In remarkable circumstances it was only re-discovered recently and publicly released. And there is a story there, which we’ll get to later. This is one of the oldest Cockpit Voice Recordings to ever surface from a crash. The recording sheds further light into that flight deck of 55-years-ago.
What we are about to unearth and breakdown here today, are the events leading up to a fatal catastrophe that could so easily have been avoided and with the help of this recording, we’ll piece together why.

June 24, 1982 - During a calm flight to Australia, smoke starts filling the cabin of a passenger jet. At the same time, the flight crew witnesses a bizarre shower of brilliant white sparks hitting their windshield. Within moments, the four engines of the 747 shut down. Just seconds before crashing into the Indian Ocean, the engines roar back to life. The terrifying flight changes the way pilots are trained and alters forever the understanding of how volcanoes can impact passenger flights.

August 22nd, 1985 — British Airtours Flight #28 is taking off from Manchester Airport for the Greek Island of Corfu. As the aircraft is accelerating, a loud ‘thud’ emanates from the fuselage, and the pilots abandon takeoff. They believe a tire has blown, but passengers on the left side of the plane see the real problem — one of the engines is on fire. Within seconds, thick black smoke fills the cabin, panicked passengers struggle to escape, and just six minutes later, the aircraft is completely ravaged by fire. 55 people die. Can investigators determine how a minor emergency turned into one of the worst disasters in British aviation history?

American Airlines Flight 587 was a regularly scheduled international passenger flight from New York's John F. Kennedy International Airport to Las Américas International Airport in Santo Domingo, the capital of the Dominican Republic. On November 12, 2001, the Airbus A300B4-605R flying the route crashed shortly after takeoff into the Belle Harbor neighbourhood of Queens, a borough of New York City. All 260 people aboard the plane (251 passengers and 9 crew members) were killed, along with 5 people on the ground. It is the second-deadliest aviation incident involving an Airbus A300 and the third-deadliest aviation incident to occur on U.S. soil.

The location of the accident and the fact that it took place two months and one day after the September 11 attacks on the World Trade Center in Manhattan initially spawned fears of another terrorist attack. Terrorism was officially ruled out as the cause by the National Transportation Safety Board (NTSB), which instead attributed the disaster to the first officer's overuse of rudder controls in response to wake turbulence from a Japan Airlines (JAL) Boeing 747-400 that took off minutes before it. According to the NTSB, the aggressive use of the rudder controls by the first officer caused the vertical stabiliser to snap off the plane, along with the plane's two engines separating from intense forces before impact.

Because the crash occurred just two months and one day after the September 11 attacks in New York, several major buildings including the Empire State Building and the United Nations Headquarters were evacuated. In the months after the crash, rumors circulated that the plane had been destroyed in a terrorist plot, with a shoe bomb similar to the one found on Richard Reid. In May 2002, a Kuwaiti national named Mohammed Jabarah agreed to cooperate with investigators as part of a plea bargain. Among the details Jabarah gave authorities was a claim made to Jabarah by Khalid Sheikh Mohammed's lieutenant, who told Jabarah that Reid and Abderraouf Jdey had both been enlisted by the al-Qaeda chief to carry out identical shoe-bombing plots as part of a second wave of attacks against the United States. According to this lieutenant, Jdey's bomb had successfully blown up Flight 587, while Reid's attempt had been foiled.

In May 2002, a Canadian government memo was written that repeated the claims suggesting that Jdey had a role in the crash, while conceding that the reliability of the source of that information — Khalid Sheikh Mohammed's lieutenant — was unknown. According to information contained in the memo, Jdey — a naturalized Canadian citizen — was to use his own Canadian passport to board the flight. While American Airlines' passenger manifest did indicate citizens boarding with passports from the United States, the Dominican Republic, Taiwan, France,[d] Haiti, and Israel, no passengers boarded using a Canadian passport. According to NTSB spokesman Ted Lopatkiewicz, the weight of the memo's veracity was put into question, as no evidence of a terrorist traveling on board was found. The evidence suggested that the aircraft was brought down after a piece of the empennage, "the vertical fin, came off", while it did not indicate "any kind of event in the cabin."
